New Notice of Funding Opportunity for Planning Grants to Address Continuity of Care for People Who Have Been Incarcerated

Today, CMS announced a Notice of Funding Opportunity (NOFO) making available funding for planning grants to state Medicaid and Children's Health Insurance Program (CHIP) agencies for addressing operational barriers and promoting continuity of care for Medicaid- and CHIP-eligible individuals who are inmates of a public institution. This includes state prisons, local and county jails, tribal jails, and youth correctional or detention facilities.

Funds will be awarded to up to 56 recipients (states and territories) for a 4-year period of performance. Up to $106,500,000 million is available to eligible applicants. The anticipated range of awards is $1 million to $5 million per recipient over the entire period of performance.

Applicants have two opportunities for submitting applications. The application deadlines are:

  • Cohort 1: November 26, 2024, by 11:59 pm EST
  • Cohort 2: March 17, 2025, by 11:59 pm EST

All states and territories interested in applying to participate in the Section 206 State Planning Grants must submit applications to Grants.gov.
