1115 Change Request - Minnesota Reform 2020 Extension Request

On August 2, 2024, Minnesota submitted to CMS a request to extend the Minnesota Reform 2020: Pathways to Independence demonstration through 1/31/2030. The program provides home and community-based services to participants aged 65 and older who meet a nursing facility level of care and have limited income and/or assets but are yet not financially eligible for Medicaid. Services include adult companion and day services, nutrition services, chore services, home health services, family caregiver services, specialized equipment, home and vehicle accessibility modifications, conversion case management, Community First Supports and Services (CFSS), and other discretionary services. This extension also adds transitional services as a covered benefit. The federal comment period will be open from August 16, 2024 through September 16, 2024.
