Prepare Supporting Documents Request academic transcripts – Submit transcript(s) from the last degree earned and your current degree program. An unofficial transcript must include your name, your school's name, and cumulative GPA.
Request two letters of recommendation – Ask your recommenders to provide their email address for your application and they will receive an email with instructions. See page 51 of the Scholarship Program Application and Program Guidance for more information. Provide the tuition and fees schedule – Provide the information for the 2023-24 school year, or the most recent tuition and fees schedule published by the school. Complete a verification of acceptance or enrollment verification form – Contact your school registrar's office or advisor to complete the verification form.
State your case in the essay – Your response must demonstrate your motivation to provide health care to underserved communities citing relevant work experience and/or activities (e.g., community service, research, and internships) to demonstrate your interest.
Scan and PDF your citizenship documents – The only acceptable documents are a U.S. birth certificate, unexpired U.S. Passport, or certificate of citizenship or naturalization.
