Get Started - Prep Your NHSC Loan Repayment Application

When applying to one of the National Health Service Corps (NHSC) Loan Repayment Programs, it's important to start gathering necessary documents early. Applicants will have the option of submitting eligible loan information electronically or manually. Review these quick tips as you prepare your application:
Upload your most recent account statement and disbursement report even if your lender has changed.
Your loan service provider is the lender or whoever is servicing your loans. All loan documents and account statements must match the loan service provider.
Consolidated loans are eligible as long as all of the consolidated loans are eligible education loans. If you have multiple consolidated loans, enter each one separately and enter the information requested for each one.
Screenshots of loan documentation are accepted as long as they contain all the needed information and are legible. A JPEG is also accepted as long as it can be opened and is legible.
You can receive a copy of your Student Aid Summary Report from the Department of Education website.
You also have the option of submitting the eligible loan information electronically if you only have federal loans.
