| Consult Our Application Checklist Review these tips as you prepare your Nurse Corps Scholarship application! | - Complete your verification of acceptance or enrollment verification form. Contact your advisor or school registrar's office for the correct email address to enter into your application.
- Email the Department of Education for a PDF copy of your official 2022-2023 Student Aid Report for your application. You can request updates on your report by calling 1-800-4-FED-AID (1-800-433-3243).
- Request academic transcripts from qualifying schools you have attended. If you have not completed one full academic term, you must submit an official or unofficial transcript from your most recent school. The transcript must be legible and your school's name, courses taken, grades received, and cumulative GPA.
- Reach out to two recommenders for a letter of recommendation. Provide their email address in your application and they will receive an email with instructions. See pages 35-37 of the Application and Program Guidance for more information.
- Review the essay question. Your response must demonstrate your motivation to provide health care to underserved communities citing relevant work experience and/or activities (e.g., community service, research, and internships) to demonstrate your interest.
- Prepare your resume/CV. Summarize your educational qualifications, work experience, community service and language proficiency skills. List your nursing license number, license type, the name that appears on your license (if different from the name on your resume), state of licensure, and expiration date under a certification/licensure section.
- Scan your proof of U.S. citizenship document and save as a PDF. The only acceptable documents are a U.S. birth certificate, an unexpired U.S. Passport ID page or U.S. Passport Card, a Certificate of Citizenship, a Naturalization Certificate, a state-issued Enhanced Driver's License (available only in Michigan, Minnesota, New York, Vermont and Washington) and/or a Permanent Resident Card (Green Card). Please note, a social security card or marriage license are NOT acceptable as proof of citizenship.
