On September 7, 2021, Utah submitted an application to CMS to extend the state's section 1115 demonstration, entitled "Primary Care Network," which provides health care coverage for various low-income populations, for five years. The request also includes a number of changes to the demonstration, which would allow the state to: change the name of the demonstration to "Utah's Medicaid Reform 1115 Demonstration Waiver, extend the period of intensive stabilization services from the first 8-weeks, to the entire period of the intensive program, and combine the four populations eligible to receive coverage under Utah's premium Partnership for Health Insurance (UPP) into one demonstration population. The state also requests a number of technical changes to change intensive stabilization services staff references and remove references to services that are no longer provided under the demonstration. The federal public comment period was open from September 21, 2021 to October 21, 2021.

Due to a technical error with the closing date of the first federal public comment period, CMS is providing a second 30-day federal public comment period from October 28, 2021 through November 27, 2021.
